New South Wales Children's Guardian Steve Kinmond has been removed from office after an independent investigation.
The inquiry was launched into the workplace culture at the Office of the Children's Guardian (OCD) and it heard from 70 current and former staff.
Communities Minister Kate Washington has told parliament the investigation found Mr Kinmond intervened in a "working with children check" while failing to declare a conflict of interest.
"The inquiry commissioner found this conduct to be a significant transgression of proper and appropriate boundaries, that risked the interests and reputation of the Office of the Children's Guardian," she said.
