Former New South Wales premier and senior Optus executive Gladys Berejiklian refused to answer questions about the major outage to triple zero services which resulted in the death of four people.

Optus CEO Stephen Rue announced on Friday that the nation’s second largest telecommunications company had experienced a major 13-hour outage on Thursday which impacted hundreds of triple zero calls in South Australia, Western Australia and the Northern Territory.

Mr Rue, who waited a day and a half to announce the news, said the substantial "technical failure" to triple zero services occurred during a routine firewall upgrade and that 600 customers were impacted resulting in the death of three people, which has since risen to four.
