ETNA, Ohio — A truck driver's failure to respond to slowed traffic on Interstate 70 in Central Ohio triggered a deadly chain-reaction crash involving a chartered school motorcoach, two passenger vehicles and another truck, according to a report released Wednesday by the National Transportation Safety Board.

The Nov. 14, 2023, crash occurred at 8:47 a.m. when a westbound truck operated by Mid-State Systems Inc. failed to slow while traffic was baked up behind an earlier minor accident. The truck slammed into the rear of the line of vehicles, sparking a fire that spread across multiple vehicles.

Three Tuscarawas Valley High School students aboard the motorcoach and three occupants of a passenger vehicle were killed. More than 40 others were injured.
