PLATTSBURGH, N.Y. (ABC22/FOX44) - UVM Health - Champlain Valley Physicians Hospital announced Friday that they are accepting applications for "innovative, grassroots solutions to some of the North Country's most pressing health challenges."

Applications will continue to be open for two more weeks, with a deadline of Friday, October 10.

Nonprofit organizations in Clinton, Franklin, and Essex Counties in New York are welcomed to apply for grants at the link here, which are meant to support community needs, including encouraging lifestyle changes to prevent chronic disease, as well as preventing drug abuse.
