The Queensland government is investigating a "bungled IT system" which could have led to child safety information going missing or not being provided to case workers, as the child safety minister warns of serious consequences.

An audit of a program called Unify, which has been used by the Department of Child Safety since April, will begin on Monday, after staff reported issues of not being able to access and input timely and accurate data.

Child Safety Minister Amanda Camm said she was briefed on Wednesday about the issue with the "significant" program, which contained data about almost 16,000 children currently in the state's care, as well as those previously known to the department.

"There is missing data that hasn't migrated, or if it has migrated, we don't know where it is," Ms Camm
