MADISON COUNTY, Arkansas — A Pennsylvania man died after a head-on collision on Highway 412 in Madison County early Saturday, according to an Arkansas State Police (ASP) fatal crash report.

Vasquez Wilfredo, 37, was reportedly driving a Honda Accord west on Highway 412 when he crossed the centerline and struck a Subaru Forester head-on at around 2:54 a.m. on Sept. 27.

The report says Wilfredo died after the crash, and the driver of the Subaru was injured and taken to a nearby hospital.

ASP said the roads were dry and weather conditions were clear at the time of the crash.
