A Sunday service at a Church of Jesus Christ of Latter-day Saints chapel turned into tragedy on September 28 when a gunman launched a violent assault that left five people dead and at least eight others injured, including children.

Authorities identified the attacker as Thomas Jacob Sanford, 40, a U.S. Marine veteran. Investigators say Sanford rammed his truck into the chapel before opening fire with an AR-15-style rifle and a handgun. He then reportedly poured accelerants inside the building and set it ablaze.

Two victims were killed by gunfire, while three others perished in the fire, police said. Fire crews later discovered three undetonated improvised explosive devices inside the wreckage.
