An actress who appeared in the pilot episode of a new ABC series has died two weeks after the premiere.

Isabelle Tate played an attendee at a bachelorette party that was involved in an accident in the first episode of 9-1-1: Nashville on Oct. 9.

The 23-year-old’s agency confirmed her death on Facebook , writing that they are “deeply saddened and completely heartbroken.”

“I’ve known Izzy since she was a teenager, and she recently returned to acting. She booked the first series she auditioned for, 9-1-1 Nashville. She had a wonderful time,” McCray Agency said, noting she died on Oct. 19. “I was so lucky to know her, and she will be greatly missed by so many.”

9-1-1: Nashville is the second spin-off of the 9-1-1 franchise, following 9-1-1: Lone Star .
