The last private mental health hospital in southern Tasmania failed a regulatory audit before its chief executive officer was fired and the board attempted to wind up the clinic in the months that followed, documents seen by the ABC show.

A copy of the 28-page audit report by the Department of Health's Regulation and Licensing Unit, obtained by the ABC, highlighted compliance problems at The Hobart Clinic.

One "immediate" issue identified called on the clinic to complete an audit of potential hanging points within five days, as "nil evidence of an audit having been undertaken was provided".

The June 5 licensing audit also made 11 "strong recommendations" to be implemented by the end of July this year, including:
