WILMETTE, Ill. (WLS) -- Police are searching for an endangered teen who went missing in the north suburbs on Tuesday afternoon.

Illinois State Police said 13-year-old Jacob Pasechnik, who disappeared in Wilmette, has a condition that puts him in danger.

Pasechnik was last seen at 14th Street and Wilmette Avenue around 3:15 p.m., police said. He was wearing a dark blue sweatshirt, a yellow shirt and black pants.

Pasechnik, who has brown hair and brown eyes, is 5-foot-5 and weighs about 120 pounds.

Police asked anyone who sees Jacob to call 911 or the Wilmette Police Department at 630-234-9001.

Officials did not immediately provide further information.
