In July 2022, three workers were in the mine repairing the roof support, which involved using a roof bolting machine weighing about 50 kilograms.
A train had taken the men down to the work location and returned to the surface.
Later, the train driver was asked to transport a tyre to the bottom of the pit.
The driver did not inform the work crew he was entering the mine because he had assumed the maintenance period had ended.
