The death of former American bodybuilding champion Hayley McNeff in August at age 37 sent shockwaves through the fitness community — and now the cause of her death has been revealed.

According to a TMZ report on Monday, McNeff’s cause of death was a drug overdose. The outlet reported that the Massachusetts Executive Office of Public Safety and Security confirmed her death was caused by acute intoxication from a deadly mix of heroin, fentanyl, 4-ANPP, cocaine and norbuprenorphine.

The manner of her death was officially listed as accidental.

An obituary posted on Boston.com described her death as “unexpected but peaceful” while her family confirmed to that she died at her home in Sudbury, Mass.
