The Staff Selection Commission (SSC) has revised the formats for disability certificates in line with the guidelines issued by the Department of Empowerment of Persons with Disabilities (DEPwD). Under the revised system, Form 5 will be used for candidates with a single disability, while Form 6 will apply to those with multiple disabilities.
