SHREVEPORT, La. – A recently released inspection report obtained by KTBS reveals what investigators describe as “across-the-board failures” at an assisted living facility in Shreveport.

Documents from the Louisiana Department of Health outline a long list of violations at The Gables at Springlake, including missing safety postings, loss of residents’ rights, unsafe food-handling practices, incomplete transportation records, medication oversight failures, and residents left with no way to report abuse or even know their rights.

Administrators repeatedly acknowledged that key requirements were not being done — problems inspectors called broad, repeated, and systemic, according to the state report.
