Reverend Jesse Jackson is in stable condition at a nursing facility and is expected to return home soon, his family said Wednesday.

The civil rights icon was discharged from a local hospital in late November, two weeks after he was admitted for care for Progressive Supranuclear Palsy, an illness he was diagnosed with earlier this year.

“Reverend Jackson was discharged from Northwestern Memorial Hospital. He is currently receiving care at an acute nursing facility where he continues his recovery,” said son and family spokesperson, Yusef Jackson. “We expect that he will be home soon. Thank you for your continued prayers, well wishes, blessings, and thoughts.”

Jackson was released from an intensive care unit on Nov. 17, and discharged from the hospital on Nov. 24.
