YOUNGSTOWN, Ohio (WOIO) - The National Transportation Safety Board (NTSB) has completed their investigation into the 2024 Chase Bank explosion that killed one bank employee and injured seven.
According to the NTSB, their investigation shows a gas line that was supposed to be shut down in 2015 was still running when it was cut by a construction crew, ultimately leading to a gas release and the explosion.
Enbridge Gas Ohio, the company that operates the line, had a work order placed to shut down the line in 2015. The NTSB could not find any paperwork that confirmed the line was shut down, despite the fact that it was marked that way in their system.
