The unlikely run of the Cleveland Guardians will continue on to the MLB playoffs as AL Central Division champs .

And Brayan Rocchio made sure they went into the playoffs with another remarkable win.

Rocchio — whose season included being sent down to Triple-A before returning to play a big role in their run — hit a walk-off three-run home run to lift the Guardians to a 9-8 win Sunday, Sept. 28 .
