PORT ARTHUR, Texas — Five months after a scaffolding collapse killed three workers at the Port Arthur LNG site in Sabine Pass, Bechtel has released the findings of its internal investigation, revealing a series of safety lapses and announcing sweeping reforms intended to prevent similar tragedies.
The April 29 collapse occurred at 1:44 a.m. while a five-person crew was performing a standard tank-building procedure known as formwork jumpingm which is a process that lifts and repositions a multilayered formwork system higher up the tank to prepare for the next pour of concrete. The crew had successfully completed eight sections during their shift and was setting the final section when the formwork gave way.
