Engineering flaws led to the implosion of a submersible which killed five people near the wreckage of the Titanic in 2023, a report has found.
US agency the National Transportation Safety Board (NTSB) said Titan owner OceanGate's engineering process for the sub was "inadequate and resulted in the construction of a carbon fibre composite pressure vessel that contained multiple anomalies and failed to meet necessary strength and durability requirements".
It also said in its final report into the incident which was published on Wednesday that the firm failed to adequately test the Titan and was unaware of its true durability.
