A US investigation has concluded that the fatal implosion of OceanGate’s Titan submersible during a dive to the wreck of the Titanic was caused by “inadequate engineering” and a failure to test the strength of its pressure vessel.
The National Transportation Safety Board (NTSB) reported Wednesday, Oct. 15, that the carbon fiber structure suffered “delamination damage” during earlier dives, which went undetected because the company’s monitoring system and analysis were “flawed.”
The Titan, valued at about $4.2 million USD, imploded on June 18, 2023, about 372 miles southeast of St John’s, Newfoundland, killing all five people on board, including OceanGate chief executive Stockton Rush.
