LOUISVILLE, Ky. —
The National Transportation Safety Board (NTSB) released a preliminary report on Thursday indicating that fractures and fatigue cracks may have contributed to the UPS plane crash at Louisville's Muhammad Ali International Airport, which claimed 14 lives, including three crew members, on Nov. 4.
The report, spanning 12 pages, includes new security camera images showing UPS Flight 2976, bound for Honolulu, catching fire during takeoff. Investigators are focusing on the engine pylon mount from the left wing, as frame-by-frame images depict the plane's left engine separating from the aircraft after an uneventful taxi and takeoff roll.
