Christmas came early for members of Oxford County Paramedic Services, who received provincial recognition for their Remote Care Monitoring and Chronic Disease Management Program.

The team, along with its allied partners, was recognized with the Ontario Health System Quality & Innovation Award – Category 2: Patient-Reported Experience Measurement, during a Nov. 25 ceremony in Toronto.

Launched in August 2022, the Remote Patient Care Monitoring and Chronic Disease Management program allows paramedics with specialized training to provide in-home health checks and remotely monitor the health condition of community members who frequently call on emergency services.
