A report covering recent activity relating to the provision of home care services in North Lanarkshire has been formally approved by councillors.
The current operating model for home support was adopted in 2018 and focuses on five main elements, including hospital discharge, same day responses and reablement, specialist support, for example for people who need end of life care, ongoing paid support and quality assurance to improve standards of care.
An important aspect is maximizing people’s independence by working on specific tasks and confidence, which is done by various types of home support staff in cooperation with specialists such as occupational and physical therapists, social workers and also families.
