PORTLAND, Ore. — Oregon's Department of Human Services (DHS) will expand training for foster parents after a 4-month-old infant died face-down in a padded bassinet, according to a state investigation report.

The foster mother reported the death May 16, and investigators determined an unsafe sleeping item — a foam-padded DockATot — could have contributed to the child's death, according to a final report from Oregon's Critical Incident Review Team.

The team investigates incidents resulting in the death of a child in the foster care system.

The infant was found face-down in the DockATot soft bassinet less than an hour after the foster mother said she last checked on the baby, the report states. The child was declared dead at the hospital about 40 minutes after being discovered.
