HASTINGS, Neb. (KSNB) - The South Heartland District Health Department is focusing on the future of community health in the four-county area, with the launching of a six-year community health improvement plan.

Several community members from Adams, Clay, Nuckolls and Webster counties were on hand Friday during the meeting to launch the plan.

The plan will take on three priorities for the District, based on the Department’s feedback from the community, which include mental wellbeing, chronic disease conditions and senior health. The main objectives of the plan include:

Mental Wellbeing:

Improve access to mental health care

Decrease prevalence of factors contributing to poor mental health outcomes

Chronic Disease Conditions:

Improve understanding/access to screening and prevention reso
