Actress Isabelle Tate’s cause of death has been confirmed.

The “9-1-1: Nashville” actor died of “a rare form of Charcot-Marie-Tooth disease” on Oct. 19, Tate’s talent agency, The McCray Agency , revealed in an Instagram Story post on Thursday.

She passed away “peacefully” and her family “requests privacy as they deal with this sudden and shocking loss.”

The agency also included a link to donate to the CMTA, the Charcot-Marie-Tooth Association.
