A new report evaluating how well hospitals protect patients from preventable harm has given several Long Island facilities top marks.

The Leapfrog Group released its Fall 2025 Hospital Safety Grades on Thursday, Nov. 13, rating more than 3,000 hospitals nationwide on their performance in preventing medical errors, infections, and injuries. The nonprofit assigns hospitals a letter grade from A to F and updates the scores twice a year.

Ratings focus exclusively on safety issues that harm or kill patients in hospitals, including staffing problems, lapses in infection prevention, medication errors, and complications during care. Leapfrog notes that such preventable issues affect roughly one in four hospital inpatients nationally.

Leah Binder, president and CEO of The Leapfrog Group, said the fall report includes a new analysis examining whether hospitals that belong to larger health systems perform better on safety. According to the group, 94 percent of hospitals earning an A grade this fall are system-affiliated, along with 95 percent of “Straight A” hospitals.
