The Navy has completed its investigation into four incidents that took place during the Harry S. Truman Carrier Strike Group’s “significantly stressful deployment” between September 2024 and May.

The first took place on Dec. 22. The Ticonderoga-class guided-missile cruiser USS Gettysburg shot down an F/A-18F over the Red Sea due to what the Navy called miscommunication and a misunderstanding between the forces.

The second involved a collision between the Truman and a merchant vessel near Egypt in February. The Navy deemed the collision “avoidable.” The third and fourth incidents involved the losses of two F/A-18s and one tow tractor off the Truman due to human and communication errors.

None of the incidents resulted in major injuries.
