Cal OSHA has issued $221,000 in penalties against San Francisco-based Devastating Pyrotechnics following the deadly Esparto fireworks explosion that killed seven people and injured two others on July 1st. The agency announced 15 citations last week, including three accident-related violations: failure to establish an injury prevention program, failure to train employees for emergency situations, and failure to train workers on fire hazards associated with the materials they handled.
