CHARLESTON, W.Va. (WSAZ) -A federal audit found West Virginia failed to comply with critical requirements while investigating reports of child abuse and in more than 90% of cases reviewed , according to a 30-page report.
The audit, conducted by the U.S. Department of Health and Human Services Office of Inspector General, examined 100 reports of child abuse and neglect from October 2023 through September 2024 during the Justice administration.

The state did not meet intake, screening, assessment and investigation requirements in the vast majority of cases, the report found.
