The National Transportation Safety Board has released its 259-page final report on the 2024 collapse of the Francis Scott Key Bridge that details the events that officials determined were the probable cause of the loss of electrical power.

The agency held a hearing last month to announce its investigation, which found that a loose wire on the DALI cargo ship caused the power failure that led to the bridge collision and collapse, killing six road workers.

Investigators also discovered that the cargo ship’s high-voltage breakers were set to manual mode instead of the automatic mode, which would have shortened the initial blackout and allowed the ship’s crew to warn the workers on the bridge.

The NTSB report also makes a number of safety recommendations.
