AUSTIN, Texas (KLTV) - Texas parents can access a downloadable form to exempt their children from public school-required vaccinations, due to House Bill 1586 becoming law.

Before this, parents had to wait for the Texas Department of State and Health Services to mail the form to them.

The form does require notarization, and once notarized, the child’s parent or legal guardian can submit the form to the school for enrollment purposes.

The same form can then be used for two years before a new one will need to be completed

This new method of managing vaccine exemptions comes as the state health agency publishes a document that lists vaccine-preventable diseases, the effectiveness of vaccines and any possible side effects.
