A review into how a live electrical cable came down on top of a Sydney train plunging the network into chaos for more than 24 hours has found the overhead wire had been identified as a risk years earlier.

In May, the power connection device on a train between Strathfield and Homebush collided with overhead wires, bringing down a 1,500-volt electrical cable on the carriage.

The system failure at this critical junction for the rail network meant power to the six tracks between Strathfield and Homebush had to be isolated.

Three hundred passengers from the train were quickly escorted out of the carriages by Sydney Trains response teams.

Commuters who were on at least two other trains on other lines also had to be escorted off due to the power isolation.
