KNOX COUNTY, Tenn. — A state report obtained by WBIR assessing the Richard L. Bean Juvenile Service Center Tuesday outlines ongoing problems and lapses at the facility and includes changes recommended to fix them.

The assessment was ordered this summer after whistleblowers came forward about concerns over medicine management and the safety of young people at the center. Jim Hart of the University of Tennessee's County Technical Assistance Service conducted the report. The 98-page assessment evaluated the facility's operations, staffing, compliance and overall conditions.

A committee of Knox County commissioners appointed to review juvenile center operations was set Tuesday afternoon to review the CTAS report and recommended changes.
