The Department of Work and Pensions (DWP) spent five years demanding a single mum caring for her autistic son pay a £5,000 debt she didn't owe after failing to correct its records.

Sarah McKenzie felt 'suicidal' when the letters first began to appear, intended for another woman with the same name, and began to feel an 'immense stress' as she cares for her son, Adam, 23.

"For five years, I've been getting these letters demanding money. It has this other woman's national insurance number on the letters," Sarah explained, adding that "Every time I call the DWP to complain, they confirm I don't owe anything, and I ask them to stop the letters.

"Our national insurance numbers are the only difference; it's a case of mistaken identity."
