An HMP Durham prisoner took own life just days after his release for a harassment conviction. David Lancaster, 42, died on November 30, 2022, and had a long history of offending , being in and out of prison for violence-related crimes .
A Prisons and Probation Ombudsman (PPO) report says that Lancaster experienced depression and anxiety, and was diagnosed with post-traumatic stress disorder (PTSD). Lancaster had previously attempted suicide on several occasions but the PPO investigation found that there was "little to indicate that Mr Lancaster was at heightened risk of suicide in the time leading up to his death."
He was last seen by a friend at around 4am on November
