A five-year-old boy who collapsed and died just hours after drinking milk was failed by his school and investigating police, an inquest has found.

Benedict Blythe died in 2021 when he was exposed to cow's milk and had a deadly allergic reaction, throwing up twice in class before collapsing and later dying in hospital. The youngster, who was allergic to the substance, had been handed a contaminated glass of oat milk, despite a strict plan devised by his parents to keep his drink free of contaminants.
