BRAINERD — Brainerd Carefree Living’s license was both suspended for 90 days and revoked by the Minnesota Department of Health.
The license suspension and revocation were issued Tuesday, Dec. 9, after the health department conducted numerous investigations into Brainerd Carefree Living and found the assisted living facility was not in compliance with several state regulations and laws, including: neglect of residents; improper or lack of documentation on medications, records, assessments, procedures, programs and requirements; and not following through with correction orders.
